本文报道了临床药师参与1例三阴性乳腺癌患者合并右侧颈内静脉血栓形成（IJVT）的治疗过程。患者于术后植入输液港，并在放疗后口服卡培他滨维持治疗期间血栓形成，临床药师分析了颈内静脉血栓形成的可能原因，参与制定了抗凝方案，并提供个体化药学服务。在兼顾抗肿瘤治疗的同时，抗凝治疗过程顺利且疗效显著，患者复查未见明显复发转移征象。

Abstract:

This article reported the involvement of clinical pharmacists in the treatment of right internal jugular vein thrombosis (IJVT) in a patient with triple-negative breast cancer. The patient was implanted with venous access port after surgery, and thrombosis occurred during the maintenance treatment with oral capecitabine after radiotherapy. Clinical pharmacists analyzed the possible causes of internal jugular vein thrombosis, participated in the formulation of anticoagulation plans, and provided individualized pharmaceutical care. Finally, while taking into account anti-tumor treatment, the anticoagulation was successful and effective, and no obvious signs of recurrence and metastasis were found in the cancer reexamination.
