抗体偶联药物（ADC）通过“靶向抗体-连接子-细胞毒载荷”三位一体的设计，实现了精准杀伤肿瘤细胞并降低全身毒性的目标，在乳腺癌治疗领域取得里程碑式突破。目前已有3种ADC被批准用于乳腺癌治疗，包括靶向人表皮生长因子受体2（HER2）的T-DM1和DS-8201a，以及靶向滋养层细胞表面抗原2（TROP2）的戈沙妥珠单抗（SG）。当前ADC研发主要聚焦于靶点多元化技术、双表位抗体工程化改造及双载荷递送技术。本文系统综述了新型ADC在乳腺癌精准治疗中的转化研究进展，旨在为新型ADC的临床应用提供指导。

Abstract:

Antibody-drug conjugates (ADCs) represent a paradigm-shifting advancement in breast cancer therapy through their trinity design paradigm combining target-specific antibodies, chemical linkers, and cytotoxic payloads, enabling tumor-selective cytotoxicity while minimizing systemic toxicity. Currently, three ADCs have received regulatory approval for breast cancer treatment: trastuzumab emtansine (T-DM1) and trastuzumab deruxtecan (DS-8201a) targeting human epidermal growth factor receptor 2 (HER2), alongside sacituzumab govitecan (SG) directed against trophoblast cell-surface antigen 2 (TROP2). This review systematically examines emerging developments in ADC technology, including novel target diversification strategies, bispecific antibody engineering innovations, and dual-drug optimization methodologies. Through comprehensive analysis of translational research progress, we elucidate the evolving role of advanced ADCs in precision oncology and provide evidence-based guidance for their clinical implementation in breast cancer management.
